Patient Intake

Our intake teams are focused on serving our communities and assisting those seeking care in finding the appropriate setting to optimize their health. We engage and align SHC facilities’ clinical capabilities with the needs of referrals from providers of all types. This central intake team streamlines the intake process by utilizing e-referral and various referral systems. Data is pushed directly from the e-referral systems into our Salesforce CRM allowing us to process referrals right away. Central intake performance metrics, data and analytics are utilized to ensure expedited responses and acceptance times, optimizing referral management. With our intake teams, we efficiently manage referrals and enhance care transitions while keeping quality patient care at the forefront.

Inpatient Care Navigation Support

This talented team of Care Navigators and Customer Experience Liaisons has developed solutions and workflows complementing our resident centered care. We are engaging our residents and families as active participants in the care planning and recovery process. Our resident centered approach, which provides information and access anytime, enhances our quality outcomes. Care Navigation represents our system-wide approach to resident centered care and allows residents and families to focus on healing, recovering, and quality of life.

Discharged Residents

Our Care Navigation team understands the transition to home can be complex. Each care navigator is equipped to go the extra mile to ensure our rehab patients experience a seamless transition home. We establish critical touch points with discharged residents to assure understanding of discharge instructions and to receive post discharge care. It’s our goal to ensure that once you successfully leave our care, you stay well.

When it comes to Life Enrichment, we do things a little differently around here. We go on vacations and regular outings, host hobby clubs, hold annual Signature Olympics events and resident art shows – and on a few occasions even zip-line, parasail, and skydive!

Signature HealthCARE employs full-time Life Enrichment Directors at all of its facilities. Each of these directors is committed to revolutionizing nursing centers’ activities and working daily to fulfill our patients’ dreams and desires. A growing number of our centers are also joining the registry for the Eden Alternative, a movement promoting nursing center culture change.
